Vaccination Conset - 2nd COVID-19 Vaccination

Dear Parent / Carer

Please find attached a letter from the NHS which explains that all young people and children aged 12-15 are now being offered the opportunity to receive a second dose of the COVID-19 vaccination (Pfizer).

Vaccinations will take place at NEECA on Friday 11th February 2022. Please complete the consent form which is also attached.
